ADVANCED APPROVAL REQUEST FOR SCHOOL ABSENCE

 

Parents requesting any pupil to be excused from Mentor Christian School attendance for other than personal or family illness shall inform the office of the desired days to be excused by turning in this completed form signed by the parent or guardian at least three school days in advance of the absence.  The reason for such absence is to be given below.  Excused absences will not be granted for activities deemed to be frivolous.

 

The office records the number of days used previously and sends it to the homeroom teacher.  The teacher shall then give grade averages to the principal and the principal shall give approval or disapproval.  At no time shall absence be in excess of five school days, except when necessary make-up and tutorial service is approved by the principal.  Permission will not be given when the pupil’s grades are below average.  Permission to be excused from class will not be given during the weeks of semester exams or Stanford testing.
